North Georgia Pain Clinic is a referral-based practice. As a team, we can work together to complete your patients’ pain management treatment by utilizing past treatments, medications and test to evaluate each patient and initiate treatment for their specific needs.
We are committed to administering the best care to our patients. Please provide the information requested to assure that your patients are scheduled promptly. Please be aware that we do NOT accept records from patients.
You may fax the “Physician Referral Form” or requested information and we will contact your patient directly to schedule an appointment after the records have been reviewed and they have been approved.
